1. Identification: A valid driver’s license or other photo ID.
2. Medical Documents: If you have any medical condition that you are receiving treatment then bring:
• Full medical history.
• Records from your primary doctor or specialists.
• A list of current medications, including over-the-counter and supplements.
• Any relevant medical clearance letters (e.g., for heart conditions).
3. Corrective Devices:
• Prescription glasses or contact lenses for vision tests.
• A working hearing aid if applicable.
4. Sleep Apnea Information:
• Recent CPAP compliance report showing at least 70% usage.
• Alternatively, 90 days of CPAP data if available.
5. FMCSA Forms:
• Completed Medical Examination Report (MER) form (optional but recommended to save time).
• Any applicable waivers or exemptions.
6. Other Relevant Documents:
• If you have diabetes, bring your blood sugar readings and recent Hgb A1C results.
• For heart issues, a letter from your cardiologist stating you are safe to drive.
